HOW TO REFER A PATIENT

 

To refer your patient, you can email your referral letter to admin@theneurogroup.com.au.

 

Alternatively, you can fax your referral to (03) 8676 4926.  

 

Please ensure that your Provider Number is clearly visible on your referral, with a current contact number for your patient. 

 

All referrals will be assessed and triaged prior to making an appointment. Please provide as much detail as possible to assist in this process which can take up to 7 days due to doctor availability.

For nerve conduction studies (NCS):

Please download and complete a NCS request form by clicking here. Return the form, along with any relevant correspondence/previous tests to admin@theneurogroup.com.au or fax: (03) 8676 4926.
